Adrenal Fatigue

Reviewed by the Nimbera editorial team · Last updated May 2026

Adrenal fatigue describes a pattern of chronic exhaustion, brain fog, and mood instability thought to result from prolonged stress dysregulating the hypothalamic-pituitary-adrenal (HPA) axis and cortisol rhythms. While not a recognized clinical diagnosis, the underlying HPA dysregulation is well documented and responds to targeted lifestyle and nutritional support.

When to see a doctor

See a doctor if fatigue is severe or debilitating, you experience unexplained weight loss or gain, low blood pressure with dizziness on standing, extreme salt cravings, or if symptoms do not improve after 8-12 weeks of lifestyle changes - these may indicate Addison's disease or another endocrine disorder requiring medical evaluation.

How we rate evidence

Strong - Multiple randomised controlled trials or systematic reviews with consistent positive outcomes.

Moderate - At least one RCT or several high-quality observational studies with plausible mechanisms.

Limited - Small trials, animal studies, or traditional use with limited human clinical evidence.

Ashwagandha (KSM-66 Extract)

ingestible
A 2012 randomized, double-blind, placebo-controlled trial found 300 mg KSM-66 twice daily significantly reduced serum cortisol and perceived stress scores versus placebo over 8 weeks.
$20-$40 per month

Rhodiola Rosea (SHR-5 Extract)

ingestible
A randomized, double-blind, placebo-controlled trial published in Planta Medica found 576 mg SHR-5 daily significantly reduced burnout scores and blunted the cortisol awakening response compared to placebo in fatigued subjects.
$15-$35 per month

Phosphatidylserine

ingestible
A randomized placebo-controlled trial found 400 mg/day of phosphatidylserine complex normalized HPA axis hyperreactivity and blunted cortisol and ACTH responses to acute stress in chronically stressed men.
$25-$50 per month

Magnesium Glycinate

ingestible
A randomized controlled trial found magnesium deficiency directly elevates HPA axis set-point (raising ACTH and CRH), and 300 mg/day supplementation combined with B6 significantly reduced stress scores in severely stressed, magnesium-deficient subjects.
$15-$30 per month

Vitamin C (High-Dose)

ingestible
A placebo-controlled trial found that ultramarathon runners supplementing 1,500 mg/day vitamin C had cortisol concentrations 30% lower post-race than placebo, demonstrating significant attenuation of stress-induced HPA activation.
$10-$30 per month

Sleep and Circadian Rhythm Recovery

lifestyle
Research demonstrates that even partial acute sleep loss significantly delays HPA axis recovery the following evening, elevating late-day cortisol - confirming that sleep architecture directly regulates the cortisol feedback loop.
