Vitamin E (Alpha-Tocopherol)

Reviewed by the Nimbera editorial team · Last updated May 2026

The landmark PIVENS RCT (247 patients, 96 weeks) found vitamin E 800 IU/day was superior to placebo for improving steatosis, inflammation, ballooning, and resolution of steatohepatitis in non-diabetic NASH patients.$10-$25 per month

How to use

Take 800 IU of natural alpha-tocopherol (d-alpha-tocopherol, not dl-alpha) daily with a fat-containing meal. This is a therapeutic dose - use only under medical supervision and for defined durations.

How it works

Vitamin E is a fat-soluble antioxidant that neutralizes reactive oxygen species generated during liver fat metabolism, reducing oxidative stress-driven inflammation and hepatocyte injury that drives NASH progression.

Where to buy

Pharmacies and health food stores. Look for natural d-alpha-tocopherol, not synthetic dl-alpha. Brands: NOW Foods, Solgar, Jarrow.

Typical cost: $10-$25 per month

Safety notes

High-dose vitamin E (800 IU+) should only be used under medical supervision. Long-term use above 400 IU/day has been associated with a slightly increased risk of prostate cancer in men. May increase bleeding risk with anticoagulants. Not recommended during pregnancy at high doses.
