Kandil et al. controlled trial (n=36, 4 groups) found melatonin improved GERD symptoms including heartburn and epigastric pain, with the authors describing it as a promising therapeutic agent for GERD. How to use

Take 3-6 mg melatonin 30-60 minutes before bedtime. Some GERD-specific formulations combine melatonin with B vitamins and amino acids. Take consistently every night for at least 4-8 weeks to evaluate effectiveness.

How it works

Melatonin is produced in enterochromaffin cells of the GI tract and appears to protect esophageal epithelial cells by suppressing gastric acid secretion, increasing lower esophageal sphincter pressure, and strengthening tight junctions in the esophageal barrier. GERD patients have measurably lower nocturnal melatonin levels.

Safety notes

Morning drowsiness possible, especially at higher doses - start with 0.5-1 mg. May interact with anticoagulants, immunosuppressants, and diabetes medications. Not for use in children without physician guidance. Do not use if pregnant or breastfeeding. Melatonin is not a substitute for omeprazole or other PPIs in moderate-to-severe GERD.
