Milgrom et al. (2012) review of polyol RCTs; xylitol gum and confections significantly reduced Streptococcus mutans counts in children's plaque and reduced caries incidence when used habitually. Maternal use reduced S. mutans transmission to infants (PMID 22899692).$5-$10 for a 100-piece bag lasting 3-4 weeks at therapeutic doses.

How to use

Chew xylitol-sweetened gum (at least 1 g xylitol per piece) for 5 minutes after meals, 3-5 times per day. Xylitol mints or candies with 6-10 g total xylitol daily are the effective range studied. Use after meals when you cannot brush.

How it works

Streptococcus mutans cannot ferment xylitol to produce acid, so chewing xylitol gum creates an unfavorable environment for the bacterium. Over time, xylitol selects against acid-producing S. mutans strains, reduces their adhesion to tooth enamel, and stimulates saliva flow which buffers oral pH.

Where to buy

Xylitol gum (Spry, Epic, Xylichew) at health food stores and online. Check labels - some gums labeled "sugar free" contain sorbitol, not xylitol.

Safety notes

Xylitol is toxic to dogs - keep gum away from pets. High doses (>40 g/day in adults) can cause diarrhea or bloating. Not recommended for children under 2 without dental guidance.
