A 2023 meta-analysis of 22 RCTs (n=1,789) published in the Journal of Clinical Psychiatry (PMID 37656283) found omega-3 PUFAs did not significantly improve ADHD core symptoms overall, but subgroup analysis of trials lasting 4 months or more showed a statistically significant benefit (SMD -0.35). How to use

Take 1,000-2,000 mg combined EPA + DHA daily with a meal that contains fat to improve absorption. Choose a product that specifies EPA and DHA content separately on the label. Allow at least 3-4 months of consistent use before evaluating effect, as shorter trials showed no benefit in the meta-analysis. Refrigerate opened bottles to prevent oxidation.

How it works

EPA and DHA are structural components of neuronal cell membranes and influence dopaminergic and serotonergic signaling pathways implicated in attention regulation; people with ADHD have consistently lower blood levels of omega-3s than controls.

Safety notes

High doses above 3 g/day may mildly inhibit platelet aggregation - use caution if you take warfarin, aspirin, or other anticoagulants, and inform your prescriber. Fish oil can cause fishy burp or mild GI upset; enteric-coated capsules reduce this. Avoid if you have a fish or shellfish allergy (algae-based DHA is an alternative). Omega-3 does not interact meaningfully with stimulant ADHD medications at standard doses, but discuss with your prescriber before combining.
