Choi et al. 2004 prospective cohort of 47,150 men over 12 years (N Engl J Med, PMID 15014182) - highest meat intake associated with 41% increased gout risk; highest seafood intake with 51% increased risk. How to use

Reduce or eliminate organ meats (liver, kidneys, sweetbreads), red meat, shellfish, and anchovies. Limit beer and spirits - both raise uric acid independently of purine content. Increase low-fat dairy (protective), vegetables, and whole grains. Fructose-sweetened drinks also raise uric acid and should be avoided.

How it works

Dietary purines are metabolised to uric acid via xanthine oxidase. High-purine foods increase uric acid production. Beer adds guanosine (a purine) and also impairs renal urate excretion. Reducing dietary purine load lowers the substrate available for uric acid synthesis.

Safety notes

Crash dieting or very low calorie intake can transiently raise uric acid (ketone bodies compete with urate for excretion) - weight loss should be gradual. Purine-rich vegetables (spinach, mushrooms, asparagus) do not significantly raise gout risk and need not be restricted.
