Kennedy et al. 2016 systematic review and meta-analysis (BMJ Open, PMID 26556483) - coffee consumption associated with significantly reduced risk of liver cirrhosis (RR 0.61 per 2 cups/day), liver cancer (RR 0.56 per 2 cups/day), and liver fibrosis progression in NAFLD patients. How to use

Two to four cups of caffeinated or decaffeinated coffee per day. Both regular and decaffeinated coffee show hepatoprotective effects (confirming the benefit is not from caffeine alone). Filter coffee and espresso both work; instant coffee shows some benefit.

How it works

Coffee contains hundreds of bioactive compounds beyond caffeine - particularly cafestol, kahweol (diterpene compounds), chlorogenic acids, and trigonelline. These modulate AMPK (a cellular energy sensor), inhibit hepatic stellate cell activation (preventing fibrosis), reduce hepatic lipid accumulation, and upregulate antioxidant response elements via Nrf2 signalling.

Any regular coffee from grocery stores or cafes. Filter/drip and espresso methods retain active compounds well. French press and boiled coffee have higher diterpene content but also higher LDL-raising cafestol - filter coffee avoids this.

Safety notes

Caffeine-sensitive individuals may experience anxiety, insomnia, or elevated heart rate - switch to decaffeinated. Avoid more than 4-5 cups/day (400 mg caffeine). Not recommended during pregnancy (limit to 1-2 cups max). Coffee can exacerbate acid reflux or GERD in susceptible individuals.
