Aerobic Exercise (Low-Impact)

Reviewed by the Nimbera editorial team · Last updated May 2026

Bidonde et al. 2017 Cochrane systematic review (PMID 28636204; Cochrane Database Syst Rev, Issue 6) pooled 13 RCTs (839 participants) and found low-to-moderate quality evidence that supervised aerobic exercise improves pain, health-related quality of life, and physical function compared with control conditions over 6-24 weeks.$0-50/month

How to use

Begin with 10-15 minutes of low-impact aerobic activity (walking, water aerobics, or stationary cycling) 2-3 times per week. Gradually increase by no more than 10% per week, building toward 30 minutes most days. Starting slowly is critical - overexertion commonly triggers symptom flares. A physical therapist experienced with fibromyalgia can design a graded program.

How it works

Sustained moderate-intensity aerobic exercise promotes endogenous opioid release and increases central pain inhibition, helping to down-regulate the central sensitization that drives fibromyalgia symptoms. It also improves sleep architecture and reduces fatigue through cardiovascular and neuroendocrine adaptations.

Where to buy

No purchase needed. Community pools, YMCA facilities, and physical therapy clinics are common starting points. Many rheumatology centers offer structured fibromyalgia exercise programs.

Typical cost: $0-50/month

Safety notes

Start at very low intensity - post-exertional symptom flares are common if exercise intensity increases too quickly. Avoid high-impact activities such as running or jumping during initial weeks. Consult your physician before starting if you have cardiovascular disease, severe fatigue, or uncontrolled symptoms. Do not use exercise as a substitute for prescribed medications without physician guidance.
